Lost "blue residence card"

Lost "blue residence card"

Hi,
my wife was married to a non-eu in the UK in the year 2003. But the year 2009 they got divorced. After they got married she was sent a blue residence card from the home office. That card is lost now, but she can not remember if that card had an expiry date or if it is permanent?
Secondly, if it has no expiry date (like 5 years), how does she apply for a new one?

Would the card have the name of her x-husband on it? Or is it just for her use with only her details on it.

I presume she has been living in the UK since 2003.

There is no expiry date on documents for EEA nationals and it will only contain her details.
If she has been exercising treaty rights in the UK for more than 5 years, she can now apply (if she wish) for confirmation of Permanent Residence using form EEA3.
