Tier 1 Extension Self Employment

I am applying for my tier 1 extension under self employment.

What documents must I submit from my accountant?

Does anybody have any samples.

Also on another note, I recently filed for self employment (june) I have not recieved a request to pay Class 2 yet. Accordning to this http://www.hmrc.gov.uk/payinghmrc/class2nics.htm this will happen in October.
Will the HO understand this, what documentation shall I provide for this?

Thanks any help on this would be greatly appreciated!
