Help: cover letter for dependant child UK visa

I am applying for my baby's tier 1 dependant visa. She is 4months old. My spouse is in UK. So I have to apply on behalf of my husband.

We have arranged almost everything. My concern is Cover letter is required for baby's visa.

If so, Please send the cover letter format.
