Which form to use? HSMP JR (extension of stay)

I transferred to HSMP within the UK in Jan 2006, i.e. in scope of the judicial review (extension of stay) 8 April 2008.

My extensions were:
- Feb 2007 (valid until Feb 2011)
- Feb 2011 (valid until Feb 2014)

I am not eligible for settlement due to absences and I will be applying for an early extension.

Which form do I use?
FLR (HSMP) (http://www.ukba.homeoffice.gov.uk/visas ... ionofstay/).

Tier 1 (General)
