Tier 1 General Extension

Hi,
I am a Director of a Ltd Company and planning to apply for extension. I have following queries
1. What should be the category - Salaried or self-employed. As a director i take salary and dividend from my company.
2. Documents to be submitted as proof of earnings
a. Salary slip
b. Dividend voucher
c. Bank statement reflecting salary and Dividend
d. Letter from Accountant - Is this required?

I would appreciate if I get the details at the earliest.

I believe for limited company setup, you select employed rather than self-employed
