PBS dependant extension closed?

I last downloaded PBS Dependant application form with title:
APPLICATION FOR AN EXTENSION OF STAY AS A DEPENDANT OF A PERSON WHO HAS LEAVE TO REMAIN UNDER THE POINTS BASED SYSTEM
on 13-Feb-2012. Footer of this form is
PBS (D) Application Form (Version 07/11)
.

I could no longer locate this form on UKBA website. Does it mean that in-country extensions applications for dependants are discontinued? Or am I looking in the wrong place?

Please advise.

Thanks for sharing this. I have seen this page, but still this does not answer my question.

Can you please tell what happened to APPLICATION FOR AN EXTENSION OF STAY AS A DEPENDANT OF A PERSON WHO HAS LEAVE TO REMAIN UNDER THE POINTS BASED SYSTEM?

Anyone in similar situation? Please share your viewpoints.

Have you considered looking at the link provided by vinny??? If you look at the UKBA page he is pointing you to, you will find that the forms for dependants to extend their stay are on the right side of the page.
